Add or remove a member
The following sample document adds a fund member as part of the order form to either add or remove a fund member.
The completed order generates the following documents:
- Application for membership
- Trustee minutes regarding membership application
- Consent to Act (as trustee or director) - where applicable
- Product Disclosure Statement( (PDS)
Please note that where the individual is added as either a trustee or director, you will need to prepare additional documents related to their trusteeship which does not form part of this order including:
- ATO Trustee Declaration;
- ATO Change of details form (also required to complete when adding as a fund member).
